abstract:
EUS-guided local thermal ablation is a minimally invasive therapeutic approach increasingly applied in patients with pancreatic cancer and recently, a new experimental device, the HybridTherm probe (HTP), combining bipolar radiofrequency ablation and cryotherapy (currently not commercially available), has shown to be feasible and safe in patients with locally advanced pancreatic adenocarcinoma (PDAC). This is the case report of a 71-year-old man, with PDAC, treated by 2 ablative sessions with HTP under EUS guide during standard chemotherapy. The patient showed a good response (necrosis inside the lesion) to ablative therapy associated with chemotherapy and underwent respective surgery (Whipple pylorus preserving surgery; histology: ypT3N1, G2) and he is currently still alive after 1466 days and adjuvant chemotherapy, with a relapse along the superior mesenteric vein, 18 × 11 mm in size.
